Service Agreement

  • Payment Options:
    • Credit Card
    • ACH/Direct Debit
  • If payment is declined or invalid, the Customer will have 14 days to rectify the issue. After 14 days, a penalty fee may be applied
  • FPM maintains liability insurance for damages or injuries caused by its services.
  • The Customer is not liable for injuries to FPM personnel, except in cases of negligence or failure to adhere to outlined safety requirements.
  • Either party may terminate this Agreement with a 30-day written notice.
  • Early termination does not absolve the Customer of obligations to pay for services already rendered.
  1. Ensure the area(s) of property where technicians will be working is safe and free of hazards, including securing pets and clearing debris prior to service.
  2. Follow all instructions provided by FPM to facilitate scheduled services.
  3. Understand that inadequate preparation of these areas may result in rescheduling, cancellation, or additional charges for remedial services.
